Scholarship forms may be scanned and emailed to info@sportsinwisconsin.com or faxed to 608.226.9550. Please Put "Scholarship Application" in the subject line. It is in your best interest to call and confirm the receipt of these forms. The office number is 608.226.4780.  Scholarship funds are intended for youth upto age 18.
